Legal duties in transition planning – Considerations for adult social care and healthcare professionals

  • The relevant service provisions that ought to be considered to include, the Care Act 2014, Continuing Health Care framework and Section 117 aftercare under the Mental Health Act 1983; and
  • The role of the Court of Protection in the transition process.
